Rules and Regulations

1. To the best of my/our knowledge the school property, for use of which application is hereby made, will not be used for the commission of no act prohibited by laws. I/we understand that no alcoholic beverages, liquors, narcotics. or firearms will be used, sold, or delivered inion school premises. //we also understand that SMOKING IS PROHIBITED i11/011 school property. 

2. l, the undersigned, hereby certify that I shall be personally responsible, on behalf of my organization for any damages sustained to the school premises, furniture, grounds, or equipment occurring through the occupancy of said premises by our organization. I/we agree to abide by and enforce the rules, regulations, and policies of the Benicia Unified School District governing the use of the school premises or equipment I/we also agree to hold the Benicia Unified School District, its Governing Board, the individual members thereof, and all District officers, agents, and employees free and harmless from any loss, damage, liability, cost, or expense that may arise during or be caused in any way by such use or occupancy of school property. I/we understand that groups or persons using school facilities under the provisions of AR 1330 shall be liable for any damages to school property caused by the activity. I/we understand that the Governing Board shall charge the amount necessary to repair any damages or provide replacement of school property. I/we understand that further use of school facilities may be denied by the responsible party.

3. I/we understand that payments are due on the dates shown on the payment tab within my application. I also understand that per BUSD Board policy, new facility use permits will NOT be issued if I/we have a past-due account. I further understand that if my group's account goes unpaid beyond the due date, full payment for any future rental will be required in advance if your group desires to rent any facility within the Benicia Unified School District.

GENERAL RULES:

•      Facility users will be responsible for all damages to the facility and fields. The facilities must be left in the same condition and appearance as when initially occupied. lf rain has occurred within the last 48 hours the fields must be assessed for condition.

•      Site administrator or designee has the authority and reserves the right to rescind facility use approval at any time.

•      NO keys will be issued to facility users.

•      Applicants are responsible for all personnel fees and use fees. All estimated fees are to be paid prior to an event taking place. Failure to do so will result in denial of the facility use application.

•      Facility users must have an approved Facility Use form in their possession during each approved activity.

•       School activities have priority over an outside request. Such priority may occur up to three days prior to the date of an approved outside event.

•      Cancellation policy will be in accordance with AR 1330.

•      A post-activity evaluation of the area may be conducted and determined by site personnel.

•      No tobacco products, intoxicants, alcoholic beverages, drugs and/or profane language are allowed on facility property at any time.

Sufficient supervision by a responsible adult is mandatory at all times.

•      No one is permitted to drive anywhere but on the driveway or parking lots. All vehicles must park only in designated parking areas.

Failure to follow the above procedures, guidelines, and rules will result in approval being revoked and the privilege of using the facilities will be suspended. The person signing this form accepts all responsibility for the actions and total supervision of the group, spectators, participants, etc. A group that has been suspended from any district facility will not be allowed to apply for use of any other district facility for a period of up to one (I) calendar year.

The undersigned agrees to defend, indemnify, and hold harmless the Benicia Unified School District, its Board of Trustees, officers, agents, and employees, individually and collectively, from and against all costs, losses, claims, demands, suits, actions, payments and judgments, including legal and attorney fees, arising from personal and bodily injuries, property damage or otherwise, however caused, brought or recovered against any of the above that may arise for any reason from or during or be alleged to be caused by the undersigned use/occupancy of the District's facilities.

The undersigned further agrees to provide a current certificate of insurance for liability coverage that conforms to the required limits that have been specified by the district.

Accepting this agreement will constitute a contract and acceptance of these required terms. Prior approval, a current insurance policy, and a Facility Use Application must be provided to the Benicia Unified School District and be approved by the District Administration.

Fire & Earthquake Response

Interior Location (Gym, MP room, PAB,
Student Center, classroom, etc.)

All persons are to exit the building
immediately. Announce to all those
present that they are to follow you.
Follow the evacuation route (or
secondary route if the primary route is
unsafe). Maps of evacuation routes
are posted inside of each
room/facility. If no map is present,
then take the safest route possible off
of campus.

Once off campus, ask those present
to stay near you and take attendance,
if possible.

If you see smoke/fire, contact Benicia
Fire at 707-745-3411 or dial 911.

Exterior Location / Athletic Fields

Direct all persons present to gather in
an open, safe area away from
structures. Take attendance, if
possible.

If you see smoke/fire, contact Benicia
Fire at 707-745-3411 or dial 911.

Contact the VP in charge of facilities

happened.

Shelter in Place

Interior Location (Gym, MP room, PAB,
Student Center, classroom, etc.)

You will want to call a shelter in place if
a non-human threat is present or
developing (air contamination,
threatening animal or dangerous
weather)

If anyone is outside, direct them into
the building.

Close doors and windows as quickly
as possible.

If possible, research conditions
(weather.com or purpleair.org) to best
plan next steps. If conditions allow for
travel, cancel your event and direct
guests to leave campus. If conditions
require you to remain, then direct
guests to stay calm and away from
windows.

Exterior Location / Athletic Fields

You will want to call a shelter in place if
a non-human threat is present or
developing (air contamination,
threatening animal or dangerous
weather)

Direct everyone into the nearest
building or vehicles. Contact the
custodian on duty to have them open
a facility for you.

Make sure all windows and doors are
closed.

If possible, research conditions
(weather.com or purpleair.org) to best
plan next steps. If conditions allow for
travel, cancel your event and direct
guests to leave campus. If conditions
require you to remain, then direct
guests to stay calm and away from
windows.

Bomb Threat 

Interior Location (Gym, MP room, PAB,
Student Center, classroom, etc.) 

Exterior Location/Athletic Fields

If your event receives a bomb threat, remain calm.
Contact Benicia Police at 707-745-3411 or dial 911. Follow any directions they provide.
Assemble your event's leadership, if applicable. Make an announcement that due to
unforeseen circumstances, the event must end and that everyone is to leave
immediately.

Contact the VP in charge of facilities Mr. Jacob Smith at 707-297-5138 or the Director
of Maintenance and Operations Mr. Alfredo Romero at 707-974-8488 and report what
has happened.

Lockdown

Interior Locatlon (Gym, MP room, PAB,
Student Center, classroom, etc.)

You will call a lockdown when you
have a reasonable concern of a
possible threat becoming present at
your event, or if a concerning event
has begun to develop.

Contact Benicia Police at
707-745-3411 or dial 911. Follow any
directions they provide.

Reassure everyone and try to reduce
anxiety. Make a plan with those
present if the situation escalates.

Discuss ALL options:
o How will you evacuate?
o How will you respond?
• Who is responsible for what
action?

Be ready to Run/Hide/Fight

After the situation is over, contact the
VP in charge of facilities Mr. Jacob

of Maintenance and Operations Mr.

report what has happened. 

Exterlor Location/Athletic Flelds

You will call a lockdown when you
have a reasonable concern of a

IMPORTANT: Only utilize the Run/Hide/Fight response when you have confirmed the active
state of a shooter or violent intruder.
* Running away from the event/area is the best option, if possible.
* Take cover in a nearby building, bathroom, or vehicle, if necessary.
* Keep everyone quiet and off of their cell phones. Stay focused on what is happening
around you. If you encounter the shooter/intruder and have no other option, fight.
Dial 911
